Mine was glossy black too and received it something like a month or two after car purchase (July last year). I was also sent a survey form (well, a link to the web based survey form) by Skoda Aus after my 15k service.

Seriously Skoda Aus, WTF! Treat your customers well. I received mail asking to fill in a survey, before I was even mailed the confirmation of my extended warranty. And I did the survey too... Made me feel like a sucker.

I got two letters about the survey.
One was not long after I brought the car home... (Confirmed and signed papers on 22nd, brought home 23rd, received on 27th)
Second one was after 15,000 service. Some time in March, I think.
Haven't got extended warranty confirmation. :S
That being said, I haven't looked through my 'goodie-bag' (manual etc) properly, so...
Yes I've read the manual, just not really anything else. XD

Hell, even though I'm currently running my 4th Body Control Module, I still recommend the Octy to everyone who asks. XD
I love this car, and the Moss Vale dealership.
Haven't had to deal with corporate... Yet. >.<
